Pensacola, Fla. – The Florida Department of Health in Escambia County (FDOH-Escambia) is now providing new options for parents or guardians submitting immunization record transfers from another state or country. The new options aim to provide more access for clients with limited time or transportation by allowing clients to complete paperwork prior to mailing, faxing, or dropping off requests.

Prior to enrollment in Florida schools and child care facilities, each child is required to have a Florida Certificate of Immunization, or DH 680 form, on file documenting age-appropriate immunizations. If you are moving to Escambia County, Florida from another state or country, you must transfer your child’s complete and up-to-date immunization records.

How to Submit an Immunization Record Transfer to FDOH-Escambia:

  • Complete and submit the following to FDOH-Escambia at least three weeks prior to school registration:
  • Documentation may be submitted via fax, mail, or in-person drop off at two FDOH-Escambia locations in Pensacola.
